 What do I bring with me to my visit?

– Photo Identification: Adult patients and adults who accompany patients under the age of 18 must have a valid form of identification such as, a driver’s license, military ID, or student identification card.

– Proof of Insurance: If you plan to pay for a non-work-related illness or injury with insurance or if the patient is a minor under the age of 18, please bring proof of your insurance. If you plan to not use insurance, we offer self-pay pricing.

– Proof of Guardianship: Adults who accompany minor patients under the age of 18 must provide proof of guardianship. Please fill out the parental authorization form and bring it with you if you’re an adult who is not a parent or legal guardian.

– Medical History: Bring information about the patient’s past surgeries and medical conditions as well as a list of patient’s allergies, current prescriptions and over-the-counter medications.

When you check-in, our friendly staff will help you register if you’re a new patient or if any of your current information has changed such as insurance or address. Feel free to download and bring our registration form with you to your appointment and give it to the front desk.

 

 Do I need to make a reservation?

Reservations are never necessary, and we always welcome walk-in patients during regular office hours. Making a reservation will hold your spot in line and may speed up wait time if there are several walk-ins during the time you come into a Marque clinic. Please note if you do make a reservation, it’s based on average wait times. We understand you are not feeling well and we try our best to see you as close to your reservation time as possible.

 

 Do you accept my insurance?

We are currently in-network with most major carriers. To see accepted insurances, please visit our Insurance & Payment page if we do not accept your insurance or if you’re planning on not using any, we offer self-pay discounts and will let you know beforehand how much your visit could cost out-of-pocket before being seen.

 

 Are you going to send me to the Emergency Room?

If we determine that you may have a serious illness or we don’t have the testing equipment available to properly evaluate and treat your condition, we may recommend you go to the nearest Emergency Room. We can courtesy call the ER you would like to go to to let them know you’re on the way to help expedite your wait time.
